Health & Safety Agreement

    Pavilion is taking prudent steps to enforce the appropriate protocols to keep members, staff and guests safe, but there can be no assurance that the virus won’t be contracted. By filling out the following, the signee acknowledges the risk of either contracting the virus or transmitting it to others when entering Pavilion’s spaces.

    Based on my knowledge and personal awareness, I (the undersigned) hereby confirm the following to be true for myself and other guests attending the sales tour: *
    1. I currently do not have any symptoms associated with COVID-19
    2. I have not been exposed to anyone with symptoms associated with COVID-19 within the last 14 days
    3. I am currently not self-isolating

    I confirm that I understand the following: *
    1. Masks are mandatory and will be worn during the full duration of the sales tour
    2. Physical distance of at least two (2) metres will be required for the full duration of the sales tour
    3. Only two (2) other individuals can accompany me during the sales tour
    4. If there are any changes to my answers above, I must report them to the Director of Sales before arrival by emailing winston@alabasterhomes.ca
